Celebrity Tier is not public signup. This form is only an initial contact request. It does not create membership, prove identity, authorize public listing, or guarantee approval. Celebrity Tier is staff/admin assigned only after BBSC verifies identity and receives approval.

Do not submit private information for someone else without permission. BBSC will not automatically add anyone to the public Celebrity Members list. Public listing requires written approval and will only show approved public/stage name and approved artform/category.
